OTTAWA UNIVERSITY'S ATHLETIC INSURANCE PLAN COVERAGE
Ottawa University maintains an athletic insurance plan for our student-athletes. The policy is in place to provide benefits such as balance of services rendered copays, coinsurance and deductibles, for approved intercollegiate athletic injuries only.
Below you will find a summary and guidelines of our Athletic Insurance Plan. If you have further questions, please feel free to contact Kathy Hinderliter, athletic department coordinator, or the athletic department training staff. We will be more than glad to assist you.
SUMMARY OF OTTAWA UNIVERSITY’S ATHLETIC INSURANCE PLAN COVERAGE FOR STUDENT-ATHLETES.
- Injuries sustained by a Ottawa University student-athlete during an official intercollegiate practice or competition will be assessed and evaluated by the athletic training staff of Ottawa University. If deemed necessary, the student-athlete may then be referred to the appropriate medical provider.
- Ottawa University student-athletes who are injured during any properly supervised official practice, competition, or conditioning session will be eligible for filing an insurance claim. The athletic insurance policy is setup to provide coverage for athletic-related injuries ONLY. The policy will only cover athletic injuries sustained during the regular academic calendar year or while participating in season with eligible intercollegiate sports that may be outside the academic calendar year.
- Parents/student-athletes are financially responsible until such time as the claim is determined to be eligible under the terms and conditions of the policy. Ottawa University IS NOT financially responsible. Eligibility is determined by the athletic insurance plan carrier.
- Claim forms must be submitted within six-months from date of injury. Bills must be submitted within six-months from the date of service. Initial medical treatment must be received within 90 days from the date of the accident.
- Policy definition of: Athletic Accident: means a sudden, unexpected event those results in Injury to the Covered Person. Maximum benefit limit: $25,000.Injury means bodily Injury caused by the direct result of an Accident occurring while the Policy is in force as to the person whose Injury is the basis of the claim which results, directly and independently of all other causes, in a Covered Loss.
- The athletic insurance policy DOES NOT cover the following:
- Injuries sustained during an academic break (i.e. summer) in which no approved athletic supervision is provided, or in any activity that is not associated with a supervised intercollegiate team practice, competition, or conditioning session, including those occurring during the academic calendar year.
- A pre-existing athletic injury, eligibility must be determined by the athletic insurance plan carrier.
- Illness/sickness or congenital conditions,
- Expense for knee orthopedic devices unless prescribed for use during post-surgical physical therapy.
